Access & Support

Support Overview

Staying independent, safe and well at home is important; but finding the right services to help can be complicated. Our Access and Support staff can work with you to access the services that meet your needs and goals.

What Access and Support can do for you: 

  • Referral and application to Aged and Disability services: for people under 50 (NDIS or HACC PYP) and for people over 50 (My Aged Care- CHSP, Home Care Packages, Respite, Residential aged care) to access services:
    • To keep you well and independent
      • Personal care and health services (nursing and allied health)
      • Shopping and cooking Domestic assistance, eg cleaning
    • To keep you safe in your home 
      • Home maintenance and modifications 
      • Assistive aids and equipment eg shower chair or rails
    • To keep you connected
      • Social support groups
      • Transport
  • Link to services and support to fill in forms eg Legal services, Centrelink, Health services, Housing, Half price taxi cards etc. 
  • Organise, attend and transport to appointments
  • Support to make informed decisions and receive medical treatment
